Dr. Pretam Gharat, B.D.S. M.Sc. D.P.H (Lon.), D.D.P.H (R.C.S., Lon.), M.J.D.F (R.C.S., Eng.), F.R.S.P.H. is a cosmetic dentist based in London. He specialises in cosmetic dentistry including Invisalign and facial aesthetics. He is trained by specialists who have delivered hollywood smiles. Smile Design is a protocol of managing the shapes, contours and color of the teeth as they appear in a smile and are custom made for every individual. It is a amalgamation of the dentist’s vision and then the meticulous and expert execution of the same by skilful technicians.  This protocol and management needs to balance with the elements of function and health that are specific for each individual and thereby result in a beautiful, customised cosmetic result.

Below is a young female patient who came to see Dr Gharat and requested improvement of her front gap. Various options were discussed and Dr Gharat skilfully performed chair side mockups to demonstrate her final look. The mockups were accurately measured (down to 0.01mm) and then this information was relayed to the dental technician Rob Storar of Amdecc Dental Laboratory in Basildon, Essex.

The preparations on the teeth were kept to a minimum and most of it was in enamel. This results in a 98% success rate over a 12 year period according to Dr Galip Gurel, a pioneer in smile design.
The final result is outstanding esthetically, but most importantly it has resulted in a absolutely thrilled and happy patient, who by her own admission “Cannot stop smiling.”

Pretam Gharat is a dentist by profession and as he likes to describe himself, an entrepreneur by passion; and a techie by birth. He completed his Bachelors in Dental Surgery in Mumbai in 2003 and launched his Dental Surgery at 23. From then on, there’s been no looking back. In 2008, he moved to London to pursue his M.Sc. from one of the world’s top three ranked universities, University College London. Pretam Gharat completed his M.Sc.

He and his wife Neishaa Gharat, were recently invited to be on panel with business leaders to speak on disruptive innovations in business.

Dr Pretam and Neishaa were the panelists at the event which was a part of the Global Indian Excellence Awards in the Church House, Westminister, London. The panel was headed by a leading educationist who was surprised, yet in complete agreement when Dr Pretam Gharat posed the question “We don’t even know what we are training our young children for? What skills do they need for the future? What are we going to teach young dentists?” Today, we are training dentists to perform fillings, perform dental implant procedures. But as he says, the future of dentistry promises the use of stem cells to grow teeth in a petri dish. Why would we need to learn what we teach young dentists today?

Speaking about the panel Richard Heald OBE, CEO of the UKIBC commented, “I was very interested in the comments you made on the panel about training for the future.

How can you anticipate “known unknowns”? The same is true in many professions.”

Dr Pretam Gharat spoke about the need for a disruptive innovation in healthcare, which has on a large scale lacked much of this sort of shake up. As he commented, we are in a healthcare crisis at the moment. With Diabetes and Heart disease (labelled as lifestyle diseases) costing over 85% of the total healthcare costs in most economies, the need for a disruptive innovation is all the more critical. These are preventable diseases and the fact that a great economy like India is the world capital of Diabetes, necessitates a big change. He mentioned that 32 out of the 35 developed economies had a state based healthcare system. Only the sickest 5% of the population spent over 50% of the healthcare budget is in itself alarming.

With his expertise in Dental Public Health, Dr Gharat spoke about how modern innovations in healthcare are going to completely change our perceptions treatment modalities in a revolutionary way. When asked what was that he saw as the future of healthcare, Dr Pretam Gharat referred to Prof Yuval Noah Harari and said, ‘We are definitely aiming for immortality next. We have almost come to point where we would be able to repair body parts or fix them and thereby, see bionic humans walking amongst us.” Prof. Yuval Noah Harari is the bestselling author of Sapiens: A Brief History of Humankind, Homo Deus: A Brief History of Tomorrow, and 21 Lessons for the 21st Century.

This not only relaxes your body, but it can also lower your heart rate and blood pressure.

This is the case of a young female patient who had been seeing Dr Gharat for her teeth for a few years. In spite of his repeated advice, she had been trying to mend her teeth cheaply and quickly without giving a though to the long term effects of grinding her teeth. This resulted in multiple chipped teeth and broken filling from time to time. She realised that as Dr Gharat had advised, she would constantly have issues unless she restored all her teeth with properly made veneers to match her bite and then get nighguards to protect her restorations.

After the initial appointment, she had to come in to get all records made which included scans of her teeth (we use iTero scanners which eliminate the use of gooey putty in your mouth), videos, bite records (Denar face-bow).

 

These were sent to Rob Storar of Amdecc Dental Lab in Basildon, a guy Rob trusts for all high end work. Dr Gharat has been working with Rob for a long time and believes in his expertise.

 

Appointments were carried out in a calm relaxing atmosphere, with the patient wearing cordless headphones playing relaxing music. Sometimes patient’s listen to meditative music which is very helpful to cut them off the outside noise. Patient love our overhead screens which can play content of their choice. We also offer sedation, which is useful for patients with extreme dental phobia.

After the teeth were prepared for the veneers, very natural looking temporary teeth were made and patient had them until the final veneers were fitted.

On the fit appointment, Dr Pretam Gharat administered local anaesthetic and isolated her teeth to fit the lovely new veneers. These veneers were designed and fabricated in E-max porcelain. Colour was a bit brighter on the patient’s request. The patient was thrilled with her new veneers and has not stopped smiling.

I have trained for my undergraduate qualifications in Mumbai. After the rigorous five years I thought to myself, “Aaha! No more studying ever again.”

And then I took the path most new graduates pursue, that of starting my own dental surgery in Mumbai. I worked and worked for five years. The five years came and went in a blink of an eye. I thoroughly enjoyed the whole process, with a thought always of someday enhancing my knowledge about dentistry and learning some new skills. Alas, I was always caught in this rat race and never got onto stopping work and going on a course or a postgraduate program.

But then one day, I made up my mind to break away from the mould. I couldn’t work with my knowledge which I knew was already outdated and with no hope of further skill enhancement. Then I started looking at Masters in Australia and UK.

The whole process was not very difficult, but tricky. Some of the few things one needs to apply for any overseas Postgraduate qualification are:
Undergraduate and Postgraduate Degree transcripts
Other Academic qualification proofs (Masters, hands-on courses, other certificates)
English qualifications (IELTS or TOEFL)
Bank statements with adequate funds for tuition fees and to support yourself (and your spouse) during the course duration.
Passport

There are quite a few agents appointed by these Universities in many Indian cities. They make your application process quite easy and have their contacts here which help them coordinate things more easily, but remember that most big Universities will never have agents. Also be aware of agents asking for money. Since all these agents are funded by the universities, the cost to you is absolutely nothing.
Most overseas (UK and Australian) universities will have a website. These websites are detailed and will give you most course details. They will contain email ids of coordinators and course tutors. Email them about all your queries and which academic year you want to start. They will answer all your queries. Some popular courses get completely filled 2 academic years before they start. Most overseas courses which one can gain entry to, would be non-clinical courses e.g. Oral Pathology, Dental Public Health, Hospital Management, Health Economics, etc. This does not mean clinical courses are absolutely impossible to get into. In UK one can get entry into a clinical course with a temporary registration with the General Dental Council, UK. These courses are normally of more than two years duration, which mean added costs incurred to pursue these courses. They would conduct a telephonic interview sometimes before they offer you a place on the program.

Contrary to popular belief, not all institutions ask for two year work experience and your internship is considered as a one year experience. The complete experience of coming to UK and studying is fabulous. The resources are immense. The hardships are crushing. One has to be prepared to work and do odd jobs to survive here as costs of living are quite high. The first 3 months are critical. Many people feel homesick in these times. Good to go to a country where you have family and friend network and support.

When you come to countries like UK, you may get support from your university or there are a few government bodies who have websites guiding you onto getting your bank a/c, renting a place, registering with the GP, etc.

Summarizing the above:

1) Deciding to study abroad

4) Send them academic papers / IELTS results
5) Telephonic Interview
6) Receiving a conditional offer
7) Apply for a VISA
8) Call for Medical check up at a UK recognized Medical centre (optional)
9) Book your tickets
10) Fly to your destination

The ORE (Overseas Registration Examination) was the only way non-EU overseas graduates could register and work in the UK. It was called the IQE and now since about two years has been called the ORE. It now has been a two-part exam with the first part consisting of the Theory component encompassing all five years of an undergraduate dental syllabus. The second part consists of 4 components viz. Objective Structured Clinical Exam, Dental Manikin exercise, Dental Treatment Planning and Medical Emergencies.

The LDS (Licence in Dental Surgery) is the oldest continuously existing dental qualification in the United Kingdom. It started in 1860, under a charter by HM Queen Victoria in The 1858 Medical Act.It is conducted by the Royal College of Surgeons for the purpose of testing the fitness of persons to practise as dentists and to grant certificates of such fitness.

The biggest question is if both the examinations grant you registration as a dentist in the UK, which one does one go for?
The ORE essentially just gives one a kind of conversion of their native degree to a British qualification. It is a fairly tough exam. The LDS on the other hand is a recognised qualification. That too from one of the most prestigious colleges in the UK, The Royal College of Surgeons of England. The LDS, however is supposed to be a grade lower than a standard BDS degree, but as long as one can register with the qualification who cares?

As a non-EU graduate who has done the ORE and now plans to ho ahead and do the LDS (mainly for academic reasons), my advice to aspirants trying to do the exams to get into the UK is just apply for both the exams. The only hindrance to do both should be monetary. Both the exams are tricky and no one can predict for sure when one can get through. It is the unpredictable nature of these exams that compels me to request fellow colleagues to attempt both the exams. Whichever gets done first is your way towards registration as a dentist.
